Many of the online Bioidentical HRT Providers accept HSA Credit Cards for products, consultations and testing.

When I first started evaluating direct-to-consumer bioidentical HRT platforms, I had the exact same questions you probably do right now:

  1. Am I actually talking to a licensed physician certified in my state, or just filling out a questionnaire?

  2. Will my medical insurance cover any of this?

  3. What’s the real monthly out-of-pocket cost once the intro offer ends?

I've tried 9 different online HRT providers in the past 7 years. Be sure to see the Trusted Providers Page and My Cost Comparison Chart to determine which one might be the best fit for you.
After personally testing and reviewing the top virtual menopause clinics, here is the exact breakdown of how online HRT actually works.

How Online HRT Works: What I Learned Testing Virtual Platforms

Every single online HRT platform is different. There are a few that are similar, but for the most part they all have a unique feature or two that sets them apart. If you are willing to spend hours diving into each's website, you can likely find 75% of what you are looking for, but some of the details only become obvious when you are a real patient.

If you don't want to spend hours researching websites, you can refer to my Trusted Providers Page and the Cost Comparison Chart to see how they vary and what you can really expect from each of the nine that I've tried thus far.

Whether you have a 1:1 Consultation with an HRT Doctor, Specialist or Clinician will be dependent on the platform's model and how they conduct business.

Will You See a Real Doctor? (How Provider Networks Compare)

Here’s where things get nuanced: about one-third of these platforms offer 1:1 video consultations with real medical profesionals, while the other half handle patient care entirely through text in a secure, HIPAA-compliant portal.

If you know you have lots of questions and it's important to you to see an HRT clinician or specialist before you are prescribed any products, then consider:

If you are good with text communication and don't feel the need to meet one-on-one with an actual HRT specialist, then consider Winona, Alloy, Inner Balance, Effecty or Musely.

The True Out-of-Pocket Costs: Cash Rates & Subscription Fees

For telehealth providers that prescribe FDA-approved bioidentical options (where the specific drug, dose, and delivery method hold official FDA approval), medication costs are generally standardized. Rather than buying directly from the platform, your prescriptions are sent to your local retail pharmacy where you can use your prescription insurance.

For example, when I used platforms like MyMenopauseRx and Midi Health, my standard regimen—an Estradiol Patch, Oral Progesterone, and Estradiol Vaginal Cream—came out to roughly $99 total for a 3-month supply using traditional prescription coverage or GoodRx discount options. Keep in mind that this sits on top of the provider's consultation or visit fee, which varies based on whether they take your insurance or you use their self-pay model.

On the other hand, if you choose a platform that utilizes compounding pharmacies  (Inner Balance, Musely, Winona, Effecty and Alloy) for custom bioidentical formulations, pricing models shift significantly. Most of these direct-to-consumer platforms operate on an all-inclusive monthly subscription or flat bundled rate (which often bundles the consultation or intake form, custom formulation, and shipping into one monthly price).

Across the major custom-compounding platforms, total monthly costs typically range from $46 per month on the lower end to $199+ per month for multi-hormone or complex custom blends.

Of the platforms that I've tried, Joi Women's Wellness is the only one that requires a subscription on top of cost of medications. Their quarterly subscription includes follow up bloodwork/labs and additional 30 minute consultations to continue discussions on how you are doing and if medications are working as intended.

Joi Women's Wellness is the most thorough of the providers when it comes to follow up and testing. If you are a data geek, this is a great option for you to consider. Use code BRONSON for 50% off first set of labs and 15% off HRT medications.

If a platform's website doesn't make its custom medication pricing immediately clear, check out my Price Comparison Chart for a breakdown of what you can expect to pay for their most commonly prescribed formulations.

Image of HRT Prescriptions that I picked up at my local pharmacy
At one point I was on cocktail of three prescriptions for my HRT. Estradiol Patch, Progesterone Tablets and Estradiol Vaginal Cream. All of these could be picked up from pharmacy for a cost of about $99 for a 3 month supply.

Will Insurance Cover Online HRT? (Visits vs. Prescriptions Explained)

The only two online, telemedicine providers who take insurance and work like a traditional doctor's office is MyMenopauseRX and Midi Health. They both name specific insurance partners on their website and both write HRT prescriptions that can be picked up at your local pharmacy using your prescription insurance or a Good Rx Coupon Code.

The other online platforms don't work with insurance and you can expect you will be paying out of pocket. Most of them do take HSA credit cards, so if you have this, you can certainly utilize it for your HRT medications, consulatations and testing if that's needed.

The providers who don't take insurance, in most cases work with Compounding Pharmacies and once a prescription is received on your behalf, it will be custom dosed and created, then shipped directly to your home. Again, you can use your HSA credit card if you have one.

Each Pump Bottle of Oestra is enough for three months. You will be billed one month at a time for this from Inner Balance.

Peggy's Current Regimen

I've been using Oestra by Inner Balance for just over a year. What I like about Oestra is a few things:

  1. Only one products vs. a cocktail of prescriptions that have different dosing instructions

  2. Sleep - Oestra has made the biggest difference in my sleep. I fall asleep faster and stay asleep longer than I did on any of the other regimens

  3. Yes, it's pricey for the first six months at $199 per month, but at month 7 it's decreased to $99 per month, which is right in line with many of the others

How Does Online HRT Work? Doctors, Costs, and Insurance Explained

Finding the right online HRT platform ultimately comes down to what matters most for your budget, insurance, and personal comfort. Whether you prefer the face-to-face reassurance of a 1:1 video visit with a doctor, desire customized bioidentical HRT medications or want to utilize commercial medical insurance there is a modern HRT telehealth option built to fit your needs.
