When I took the hormone test via a blood draw, I had been on a topical HRT cream that contained both estradiol and progesterone. My blood levels barely recognized that I had been using the cream, so we soon realized, the cream, at least for me wasn't absorbing and wasn't effective. Who Can Prescribe Testosterone?


Does Joi prescribe Testosterone for Women? Because testosterone is a controlled substance (specifically a Schedule III controlled substance in the United States), it requires a formal prescription from a licensed medical professional who holds a valid Drug Enforcement Administration (DEA) registration.


Several types of healthcare providers can evaluate hormone levels and write a prescription for testosterone therapy:


1. Primary Care Providers (PCPs)

For many people, a primary care provider is the first stop. Because the symptoms of hormone imbalances often mimic other everyday health issues (like chronic fatigue, stress, or vitamin deficiencies), a PCP can run initial blood panels to look at the whole picture.  


  • Family Medicine Physicians (MD/DO)

  • Internal Medicine Physicians (MD/DO)

  • Nurse Practitioners (NPs) and Physician Assistants (PAs) (Depending on state-specific legislation regarding controlled substances)


2. Medical Specialists

If a case is complex, or if a primary doctor prefers a deeper clinical focus on specific underlying causes, they will often refer out to specialists who manage hormone health daily:  


  • Endocrinologists: These physicians specialize entirely in the endocrine system, which manages the body's glands and hormone production. 

  • Urologists: Urologists focus on the urinary tract and the male reproductive system. 

  • Gynecologists (OB/GYNs): For women experiencing specific hormonal imbalances, such as severe postmenopausal hypoactive sexual desire disorder, a gynecologist may evaluate and manage low-dose testosterone therapy when appropriate.


3. Specialized Clinics and Telehealth Providers


There are numerous specialized "Men’s Health," "Women’s Health," and longevity clinics—both physical and virtual—dedicated to hormone replacement therapies. These clinics are staffed by licensed MDs, DOs, NPs, or PAs who specialize in hormone optimization and gender-affirming care.

The Clinical Standard: A reputable provider will never prescribe testosterone based on symptoms alone. Legitimate treatment requires a comprehensive medical history, a live conversation, and accurate laboratory evidence.

Telehealth Rules for Testosterone Prescriptions


Navigating telehealth rules for testosterone can feel like tracking a moving target. Because testosterone is a Schedule III controlled substance, it is subject to strict federal and state oversight.


If you are looking into virtual care or evaluating how these platforms operate, the regulatory landscape comes down to three main boundaries:


1. The Federal Rules (The DEA & The "Telemedicine Cliff")


Historically, a federal law called the Ryan Haight Act required doctors to see a patient in person at least once before they could prescribe any controlled substance via telehealth.  


2. State-Level Restrictions Trumps Federal Rules


Even though federal rules are relaxed right now, state laws always dictate the ultimate boundaries. A telehealth clinic must comply with the laws of the state where you are physically sitting during the appointment.  

This is why early in the process the telehealth provider will ask what State you live in so they can validate if they have a provider who is licensed in your state.


3. Asynchronous Care is a Red Flag


In the world of telehealth, there are two types of visits: synchronous (live, face-to-face video calls) and asynchronous (filling out a written text questionnaire without a live call).


If a virtual clinic is operating safely and legitimately, their process should look like this:


  1. They require you to go to a local lab (like Labcorp or Quest) for comprehensive blood work.

  2. You meet with a clinician via a live video call to discuss your health history.

  3. They mandate regular follow-up blood work (typically at the 3-month and 6-month marks) to monitor your red blood cell count, liver enzymes, and estrogen levels.


If an online platform offers to ship you testosterone based purely on an online quiz without a live video call or verified lab work, they are operating outside of federal guidelines.

Is Testosterone available Over the Counter?


No, Testosterone whether its for men or for women isn't available over the counter.

How Much Does Joi Cost


Joi will not be as affordable as some of the other HRT providers you may be considering. Joi is one of the few who offer Testosterone for women, many of the others do not. When you factor in the quarterly subscription at $150/quarter/$50 per month, cost of meds which will vary and then the initial blood work. For a bit higher price tag you are getting ongoing care, medications and quarterly labs.

Are Joi Hormones Compounded?


The answer to this quesiton will depend on where your HRT medications come from. For a fee of $30 Joi will send prescriptions for Estradiol (Estrogen) and Progesterone to your local pharmacy where you can use your prescription insurance or a GoodRx coupon code.


All medications you purchase through a typical local pharacy will have FDA approved ingredients and FDA approved dosage/ratios. Doctors are limited to what they can prescribe based on available doses that have been mass produced.


If you choose to have Joi ship your medications directly to your home, then they will likely come from their partner compound pharmacy. Think of a compound pharmacy as a boutique pharmacy that custom-creates medications based on the doctor's orders and pateint's needs.


Due to the fact that the dosage and/or ratio of progesterone to estradiol will likely be custom for you, it will not have gone through the rigorous FDA approval process, thus making it not FDA approved. However, in ALL cases, Estradiol, the ingredient and Progesterone, the ingredient are both FDA approved, it's just the custom dose that hasn't been reviewed.


Testosterone isn't FDA approved for women, so when it's prescribed by Joi Women's Wellness, it's being prescribed as "off label." An off-label prescription is when a doctor legally prescribes an FDA-approved medication for a purpose, dosage, age group, or route of administration other than what the FDA officially approved it for.
