My first order of Winona inlcuded their 50/50 Estrogen/Progesterone topical cream DHEA tablets and their arousal cream. Later I also tried their prescription face cream.
Medical Disclaimer & 2026 Update: I am a patient sharing my personal journey with Bioidentical Hormone Replacement Therapy (BHRT). This review is for informational purposes and is not medical advice. Following the February 2026 FDA update regarding HRT labeling and compounding transparency, I have updated this post to reflect my latest labs and clinical discussions with my provider. Always consult a licensed professional before starting any hormone protocol.
Over the past seven years I've tried pretty much every BHRT product that is available today. Once I found BHRT care could be provided by telehealth with HRT specialists who knew their stuff, I've never looked back.
My GYN doesn't seem to have a clue on the new science so I found BHRT providers who not only understand the science, in some cases they invented the 'science.'

What Is Oestra by Inner Balance?
Oestra: The Systemic, All-in-One Vaginal Protocol ($199/month)
Rather than offering a menu of separate pills, patches, and pumps, Oestra focuses on a singular, high-potency formula: a specialized dual-action vaginal cream containing 3 mg of Estradiol and 100 mg of Progesterone.
Why Choose Oestra: It is built specifically for women who want a consolidated, targeted delivery method. Instead of taking an oral progesterone capsule at night and applying a separate estrogen gel in the morning, Oestra combines both bioidentical hormones into a single intravaginal application. This direct delivery route optimizes both localized pelvic tissue absorption and systemic relief for hot flashes, night sweats, and sleep disruptions without the hassle of managing multiple prescriptions.
What is Winona?
Winona: Maximum Delivery Flexibility on a Budget
(Starting at $89/month)
Winona operates as a broad-spectrum BHRT platform, making it one of the most accessible and affordable online options available. With plans starting at $89 per month/billed in 3 month orders, Winona allows doctors to tailor your prescription across a wide variety of formulas based on your personal preference.
Why Choose Winona: It is ideal if you want options or are on a tighter monthly budget. Winona offers standard transdermal Bi-Est creams, combined estrogen/progesterone creams, bi-weekly skin patches, oral progesterone tablets, and traditional estrogen pills. If you dislike vaginal creams or prefer the steady, set-it-and-forget-it nature of an estrogen patch paired with an oral progesterone capsule, Winona provides that level of customization.
Key Differences in Bioidentical Hormone Formulations
When comparing these two telehealth platforms, the core distinction comes down to their formulation philosophy: all-in-one simplicity versus multi-prescription flexibility.
Oestra’s Proprietary All-in-One Solution
Founded by board-certified physician Dr. Sarah Daccarett, MD, Inner Balance created Oestra to eliminate the "patchwork protocol" that plagues traditional HRT. Instead of juggling a patch for estrogen, an oral pill for progesterone, and a localized cream for vaginal atrophy, Dr. Daccarett engineered a single, proprietary dual-action formulation.
Oestra combines 3 mg of bioidentical estradiol and 100 mg of micronized progesterone into a single, pH-balanced intravaginal cream. Applied daily, it utilizes an advanced anhydrous base engineered for high bioavailability. Because vaginal tissue is rich in blood vessels, this delivery route bypasses first-pass liver metabolism—achieving nearly 100% systemic absorption compared to the 10–20% typical of oral pills or 3–20% of standard transdermal creams. Dr. Daccarett’s formulation also leverages progesterone as a natural upstream precursor, allowing your body to support its own downstream hormone pathways (including testosterone balance) without needing extra androgen prescriptions.
Winona’s Traditional, Multi-Prescription Approach
Winona takes a classic, modular approach to BHRT by prescribing standardized, traditional delivery formats. Rather than combining active ingredients into a single application, Winona relies on distinct, separate prescriptions tailored to your specific preferences.
If you choose Winona, your protocol will likely consist of two or more individual products: a transdermal Bi-Est cream or twice-weekly patch to deliver systemic estrogen, paired with an oral micronized progesterone capsule to protect the uterine lining and aid sleep. While this traditional setup requires managing multiple medications, refills, and application schedules, it offers the flexibility to adjust your estrogen dose independently from your progesterone dose—a benefit for women who want granular control over individual hormone levels.
Simplicity vs. Custom Combinations
Choose Oestra if you want Dr. Daccarett's science-backed, single-step formula that replaces multiple prescriptions with one high-absorption vaginal cream.
Choose Winona if you prefer a traditional, multi-product routine using standard patches, pills, or topical creams that can be individually adjusted over time.
"Women don't need more complexity—they need hormone care that works." — Dr. Sarah Daccarett, MD, Founder of Inner Balance

Frequently Asked Questions: Oestra vs. Winona
Is Oestra worth $199/month compared to Winona's starting price of $89/month?
Personally I found my sleep improved when I switched from the traditional patch/pills to Oestra. So for me, yes the cost difference was worth it.
What is the advantage of Oestra’s vaginal delivery vs. Winona’s patches and creams?
Oestra's vaginal delivery is simple, one time per day and only one prescription vs. Winona's protocol will likely require that you purchase two or three products.
Do I need separate progesterone pills with Oestra or Winona?
Oestra's vaginal cream includes 100 mg of Progesterone, so a separate product is not required. Winona offers their 50/50 cream which is estrogen and progesterone, so you could also get both from their topical cream.
What I noticed with the topical creams (those that you rub on your inner arms) is that I didn't feel "as great" as I did when using Oestra. However, because Winona's cream is a bi-est or combo of E2 and E3 estrogens, it's considered a more gentle dose of HRT if you want to start out slow.
Can I use my HSA/FSA card for Oestra and Winona?
Yes, both companies allow you to use your HSA/FSA credit cards.
How quickly will I see results for sleep and hot flashes?
I can't speak for everyone, but I noticed a difference in my hot flashes and night sweats in a few days, even when trying the more gentle topical bi-est creams. However, it wasn't until I switched to Oestra, that my sleep greatly improved.
The Final Verdict: Which BHRT Platform Should You Choose?
The answer for you will likely boil down to if you have a preference on the formulation you want to try and if you are adverse to having to use multiple products.
Topical creams, like that offered by Winona and are bi-est or Estriol + Estradiol vs. BHRT products that contain straight Estradiol are thought to be more gentle, but for me didn't provide the systemic complete relief of symptoms I was looking for. Once I switched to Oestra by Inner Balance which includes 3 mg of Estradiol + 100 mg of progesterone that's when I felt relief of all sympoms and saw improvement in my sleep.
