Oestra by Inner Balance: My Honest 1-Year Review

I'm 59 years old, and I've been using bioidentical hormone replacement therapy (BHRT) since 2019. In fact, it was a doctor I now jokingly refer to as my 'diet doc' who first introduced me to BHRT. I remember thinking at the time, “Wait… doesn't this stuff cause breast cancer?”
Fast-forward seven years, and I've tried seven different online HRT providers, including Inner Balance and its Oestra formulation. Along the way, I've learned a lot about hormone therapy, spent countless hours researching the science behind it, and—most importantly—I've learned that the relationship between HRT and breast cancer risk is far more complicated than the simple HRT causes breast cancer message I had heard for years.

My Oestra Experience After One Year
In July of 2025, I had been on the Estradiol Patch + Progesterone Tablets + Estradiol Vaginal cream for a few years. I felt really good, but my sleep still wasn't where I wanted it.
Although the hot flashes, night sweats, brain fog, moodiness and extra weight were under control, I was still waking up at 3:00 AM most nights. I would wake up, go to the bathroom, look at the close and will myself back to sleep. Sometimes that happend in less than an hour and somtimes it took longer.
I had slept fine as a younger women and couldn't help but wonder if something was still missing in my HRT regimen? My social media feed began to show advertising and different campaigns on Oestra by Inner Balance and I was intriged.
Being the HRT patient reviewer pioneer that I am, I thought "What the heck," I might as well try it.
On the 2nd night of using Oestra, I felt myself getting very sleepy 30 minutes within applying my dose of Oestra, vaginally as directed. I also started to notice that night and the ones to follow, most nights I stayed asleep longer and when I did wake, it was for a shorter period of time.
What Has Oestra Helped With After One Year?
My Sleep! As a 59 YO woman who understands how important sleep is, I am truly grateful for finding Oestra. Once you learn of the negative side effects of not sleeping, it's easy to go down the rabbit hole of all the bad outcomes it can have on our health.
Finding a product that treats my symptoms of menopause and helps with sleep was a clear winner for me.
Not sleeping well during the menopause years can cause short-term fatigue and long-term health risks like high blood pressure, diabetes, and memory problems.
Short-Term Effects
Daytime fatigue: Feeling extremely tired, sluggish, and low on energy during the day.
Mood changes: Higher rates of irritability, sadness, anxiety, and worsening depression.
Mental fog: Trouble concentrating, remembering things, and thinking clearly.
Headaches: Waking up with or being more prone to headaches and migraines.
Long-Term Health Risks
Heart health: Increased risk of high blood pressure and cardiovascular disease.
Metabolic issues: Higher chances of developing type 2 diabetes and struggling with weight gain.
Brain health: Greater cognitive concerns and a potential link to long-term neurodegenerative risks.
Oestra, by Inner Balance Side Effects: My Pros and Cons
To be completely honest, I wasn't exactly thrilled about the idea of Oestra by Inner Balance being administered vaginally. Why couldn't this HRT be as simple as swallowing a pill or rubbing some cream on my inner arm? The thought of using a vaginal applicator every day sounded like one more step I really didn't need.
But after a year of using Oestra, I have to admit that it hasn't been nearly as much of a hassle as I expected. From the beginning, I knew I would probably be more comfortable using a disposable vaginal applicator to make sure I was getting the cream where it needed to go. So I bought a box of 60 on Amazon, and honestly, it became pretty routine. It's certainly not my favorite part of HRT, but it's also not the inconvenience I thought it would be.
Another pleasant surprise has been the cream itself. The Ellage Cream Base, which is the third ingredient in Oestra, is formulated to stay where it's applied rather than simply leaking back out. As long as I use the bathroom before applying it, I very rarely notice any residual cream in my pajamas. That may sound like a small thing, but when you're using something every day, those little details matter.
The vaginal delivery is also one of the things that makes Oestra different from the other HRT formulations I've tried. Dr. Sarah Daccarett, founder and CEO of Inner Balance, has spent considerable time studying vaginal hormone delivery and describes the vagina as a highly effective route for hormone absorption. She has also explained that delivering hormones vaginally may more closely mimic certain aspects of how hormones were delivered to the body earlier in life.
After using Oestra for a full year, I can say that the vaginal application was something I initially worried about much more than I needed to. And while I can't say that the delivery method alone is responsible for how well Oestra has worked for me, it is certainly one of the things that sets it apart from the other HRT formulations I've tried.
One other thing to mention, because Oestra is a custom compounded medication, you will not be able to use your prescription insurance. You can use your HSA Credit card though.
How Much Does Oestra Cost After One Year?
Let's talk about the real elephant in the room: the price.
At $199 per month for the first six months and then $99 per month after that, Oestra is a significant investment—and when you're making a decision that involves your health, spending that much money can feel like a pretty big risk.
HRT costs can vary widely. Depending on the provider and what's included—such as lab work, medical consultations and ongoing support—you can find options ranging from around $33 per month to more than $200. So there's no question that Oestra is on the higher end of the price spectrum.
But there's an important piece of the equation that makes me feel better about that initial investment. Inner Balance offers a 180-day (six-month) money-back guarantee on Oestra if you don't experience a transformation or feel that you're getting results. That gives you time to see how Oestra works for you before you're fully committed to the cost.
And I think the word patience is important here. Six months is a long time to wait when you're hoping to feel better, but it may take time to really evaluate whether a hormone therapy is working for you. I wouldn't expect to know everything about how I respond after just a few weeks.
When I average the cost of Oestra over the entire first 12 months, it works out to about $149 per month—$199 for the first six months and $99 for the next six. That's much easier to compare with other HRT options on an apples-to-apples basis.
So yes, Oestra is expensive. There's no way around that. But knowing there's a six-month money-back guarantee gave me some reassurance that if it didn't work for me, I wasn't simply stuck having spent nearly $1,200 with nothing to show for it.

Is Oestra by Inner Balance Worth It After One Year?
After a full year of using Oestra, there are three things that make it worth the $99 per month I’m currently paying:
1. One prescription, one dosage schedule. Before Oestra, I was managing three different prescription products with three different dosing schedules. Having everything combined into one prescription has made my HRT routine so much simpler. And for me, simplicity matters. The fewer things I have to remember, the more likely I am to stay consistent.
2. It's bioidentical. Knowing that Oestra uses bioidentical hormones is another big plus for me. Bioidentical hormones are chemically identical to hormones produced naturally by the body. I personally prefer that approach, and knowing that the hormones in Oestra are designed to be structurally identical to the hormones my body naturally produces gives me an added level of comfort.
3. Sleep.I can't say this enough: the improvement in my sleep has been the biggest benefit for me, hands down.
After a year of using Oestra, this is the one benefit that continues to stand out. Better sleep affects everything else about how I feel during the day, and for me, that alone makes the current $99 monthly price feel absolutely worth every penny.
If I were starting over today, knowing what I know now, would I choose Oestra again? Without hesitation.
