Medical Disclaimer & 2026 Update: I am a patient sharing my personal journey with Bioidentical Hormone Replacement Therapy (BHRT). This review is for informational purposes and is not medical advice. Following the February 2026 FDA update regarding HRT labeling and compounding transparency, I have updated this post to reflect my latest labs and clinical discussions with my provider. Always consult a licensed professional before starting any hormone protocol.

I have been a patient of seven of the most popular online Hormone Replacement Therapy providers over the past two years, Midi Health included. Midi Health allowed me to attempt to use my medical and prescription insurance.

Midi Health and Insurance Coverage

Midi Health accepts medical insurance from several of the larger major providers. If your insurance provider isn't noted on their website, then you can reach out to Midi Health and they can validate if they take your insurance.

When I became a patient of Midi Health I learned that they didn't take my insurance and if I chose to move forward, the fee for the initial consultation would be $250 and would go toward my 'out of pocket' deductible for that year. If they take your insurance, the exact price you pay for the initial consultation will be dependent on your insurance, your co-pays and where are you are with deductibles in the insurance year.

Midi Health will write and send prescriptions to the pharmacy of your choice. From there you'll be able to use your prescription insurance or even a GoodRx Coupon. Midi Health only prescribes medications that are FDA approved, based on ingredients and dosage. Due to this fact, you will be able to use your insurance for most prescriptions that Midi Health may prescribe.

Does Midi Health prescribe Testosterone

When an HRT doctor or clinican prescribes testosterone for a woman, it's considered off-label. This means that testosterone has not been FDA approved for use in women. Because Midi Health only writes prescriptions that are FDA approved, they do not prescribe testosterone.

If you have done your research and want to be prescribed testosterone, you'll want to check out Joi Women's Wellness. They work with custom compound pharmacies and can and will prescribe Testosterone if it's in your best interest. Joi Wellness will request that you select a hormone lab (bloodwork), put it in your cart and pay. All Joi labs include a 30-minute 1:1 consultation to review your labs. You'll be emailed paperwork to visit your nearest Labcorp to have blood drawn once the lab is paid for.

Upon completion of the labwork, LabCorp will send results both to you and to your Joi HRT Clinician. If you find that your testosterone is low and your Joi Clinician feels it appropriate, they can prescribe testosterone off-label and have it sent directly to your home.

Upon completion of my account, I was given access to Midi Health's HIPAA compliant patient portal. I received login credentials so I have continued access to appointments and my HRT plan via the Midi Health Patient Portal.

Midi Health Patient Portal

Upon creating an account with Midi Health, you'll be given access to their patient portal. You'll be able to schedule your initial 30-minute consultation and put your insurance card on file. When I was a new client of Midi Health, I was able to get an appointment the very next business day.

Midi's patient poral is HIPAA compliant and safeguards all of your personal health information.

Image of my HRT products that I picked up from local pharmacy.
My prescriptions included the Estradiol patch, Progesterone Tablets and an Estradiol Insert. The cost for all three products for a 90-day supply was less than $100. You will also have costs associated with the initial consultation and any neede follow-up visits for refills and on-going care.

How Much Does Midi Health Cost Without Insurance?

The cost of the initial 30-minute consultation is $250 and if you have medical insurance that Midi accepts it can be used to offset this fee. Your exact out of pocket cost will be dependent on your insurance, your required co-pay and where you are with annual deductibles. I know, not very clear, but very much the same as if you visit your primary care physican, the costs vary from visit to visit.

The common HRT combo I see prescribed includes a systemic (whole body) estradiol product (a cream, gel or patch), a progesterone product (tablet) and then in many cases a vaginal estradiol insert or cream to help with dryness and vaginal atrophy. Vaginal estradiol is local and stays in the vagina and doesn't travel outside that area. If you prefer a local product and don't want a whole body estradiol product, be sure to share that with your Midi Health clinician.

The last time I picked up my combo of Estradiol Patches, Progesterone Tablets and Estradiol inserts my total cost for a 90-day supply of all three products was less than $100 or about $33 per month from my local CVS pharmacy. But don't forget, you still have to pay for the consultation and then each quarter, you'll have follow ups for refills and on-going care. Depending on the length of those appointments they can vary from $150 per appointment to $250 per appointment.

Due to the Estradiol Patch shortage of 2026, many women are finding it difficult to get new prescriptions and refills of the patch. This crisis is fueled by a massive 72% surge in demand following the FDA’s recent removal of black box warnings, combined with manufacturing bottlenecks that simply can't keep pace.

My current HRT regimen is Oestra, by Inner Balance created by Dr. Sarah Daccarett. She has discovered and proved via her science that vaginally delivery is a much improved way to get our hormones and using this delivery method is a super highway of sorts. Check out Oestra here. How is Midi Health Different from a Traditional OBGYN or PCP?

Unlike a traditional OBGYN who must balance surgical schedules and prenatal care, Midi Health clinicians specialize exclusively in midlife hormonal health, providing the dedicated time and expert-level training required to manage complex perimenopause and menopause symptoms.

This focus allows for a more personalized "whole-person" approach that often combines FDA-approved hormone therapy with evidence-based lifestyle and botanical interventions.

Can I Use My HSA or FSA Card for Midi Health?

To use your HSA or FSA card with Midi Health, simply enter your card details as the primary payment method during registration or when prompted by their billing partner, Athenahealth. Since Midi is a clinical medical practice, these charges are automatically coded as qualified medical expenses, which prevents most transaction declines and allows you to pay for consultations, co-pays, and prescriptions using pre-tax dollars.

Midi Health Review 2026 (My out of pocket costs when using insurance)

While the traditional "combo platter" of the estradiol patch, progesterone pill, and vaginal inserts is the standard protocol offered by clinics like Midi Health, it is important to remember that this approach has been around for decades with very little scientific evolution. My own journey proved that standard doesn't always mean optimal; despite being on a full regimen, my sleep remained the missing piece of my recovery puzzle. It wasn't until I looked beyond these legacy products and discovered the newer science behind Oestra by Inner Balance that I finally experienced the deep, restorative sleep I had been chasing.

Just because Midi Health is the most popular name in the space right now doesn't necessarily mean it is the best option for your specific biology. As you begin your HRT journey, it is vital to look into newer, more advanced formulations and check out reviews for products like Oestra to ensure you are choosing the most effective tools for your body. For me, stepping outside the traditional norm was the only way to fall asleep faster and stay asleep longer—and it may be the key for you, too.

Welcome

I am an HRT patient reviewer of online platforms. I've become a patient & paid out of pocket (sometimes with & sometimes without insurance) for every HRT product I've tried and review. These reviews are my honest experience as I continue to navigate menopause, just like you!
